    Abstract: 因應行政院組織改造工程陸續上路，部分部會將予以整併，組織間既有之疆界亦將打破，原有的業務流程亦將隨之重新設計。同時，伴隨著我國採用行動上網之人口比例已突破七成，行動化服務於公、私部門皆已成趨勢；儘管如此，為提供偏遠鄉鎮或弱勢族群同等品質之公共服務，近年來，透過公務人員利用行動載具提供主動化、宅配化服務，亦已形成我國公部門服務之創新模式。結合上述國內發展趨勢，本研究將以結合衛政與社政於一體之長期照護服務輸送為焦點，針對長期照顧中心之個案管理師於服務輸送及服務接受之需求面進行需求評估，探究如何從實際需求出發，向內整合行政機關內部之既有作業流程與資料庫、向外提供整合的、單一的行動介面（如Apps），俾使我國未來長照體系朝向「全面主動服務」（Total Active Services）之目標邁進。在研究方法上，本研究將採焦點團體座談及深度訪談為主要資料蒐集方法；其中將針對長照個案管理師、醫院醫師，及衛生和社福領域之專家學者，進行焦點團體座談。此外，亦將依循前述長照個案管理師之實務需求，以深度訪談方式，邀請相關主管機關共同探討整合相關資料庫與行政流程之可行性。最後，依據研究發現進行全面主動長照服務之服務規劃和策略研擬。
