    Abstract: 此刻正積極規劃與推動的長期照顧保險被譽為台灣建構完善福利國家的最後一項工程。但對位處偏鄉的原住民而言,缺乏文化能力的社會福利等同扼殺原住民靈魂的文化殖民手段,長照保險的開辦可能代表著「原鄉沒有服務,原住民卻要繳錢」、「接受服務越多,部落卻受傷越深」的雙重傷害。長期照顧在制度的設計上如何在「普遍平等」的公平原則中,保有「尊重差異」的多元彈性空間,成為長照制度設計的重大考驗。因此,本研究從原鄉部落的角度檢視目前台灣長期照顧政策的實施狀況,指出政策與原鄉部落經驗的斷裂之處,並對長照保險的規劃提出「原鄉長照由原住民自治」的建議。 本研究以介惠基金會在屏東縣原鄉執行的居家照顧服務為例,試圖運用建制民族誌的設計,分析部落在現有居家服務政策的框架中,如何走出符合部落主體性的照顧服務。本研究詳細紀錄部落實作居家照顧服務的經驗,將讓我們看到部落需求在居家服務體系下的扭曲、因應、反抗與調整,從中我們試圖分析居家服務體制背後的漢人、都會生活、與工作倫理預設,與部落文化中的共同照顧、共同勞動精神背道而馳,造成管理者與居服員之間不必要的衝突,值得目前正在積極規劃的長期照顧政策朝向多元文化精神的政策參考。
    Long term care insurance which is under process is seen as the last piece of a comprehensive welfare state for Taiwan. However, for indigenous peoples in the reserve areas, the implementation of long term care insurance could mean double disadvantages for them. The result may be that indigenous people will have to pay the same fee without the same access to services, or the services they receive deny their traditional ways of life. A welfare policy which is blind to indigenous cultures can be a means of cultural genocide. How to design a long term care system which integrates the principles of equality for all and respect for diversity is a challenge. This research is aimed to identify the gaps between the home care policy and the lived world of indigenous peoples with a vision of integrating self-determination principle into the design of long term care system. The research adopts institutional ethnography as research design to investing how indigenous communities develop home care services that reflect their ways of living. These instances of resistance offer a way to imagine alternative ways of thinking about long term care in indigenous communities.
