    Abstract: 經顱磁刺激(transcranial magnetic stimulation, TMS)是近年來引起關注的新興技術,為人所知的是在神經精神疾病的醫學用途。它逐步購置於不同地區的醫院診所,並有專門化的醫療社群。本研究採取行動者網絡理論(Actor-Network Theory, ANT),分析TMS此一醫療創新在台灣擴散的情形。以技術為中心的視角,我們採用次級資料和深度訪談的質性方法,描述TMS在不同網絡的軌跡,分析關注點和參與行動者,以了解旨趣被促動的轉譯過程。

    研究發現,(一)TMS移動於不同網絡,行動者是多重且異質的神經精神科專業,從事研究和治療、來自醫院和診所,有著零散或聚集的聯盟。(二)TMS的軌跡伴隨著關注點位移,從醫學問題移至臨床議題。隨著多重專業加入網絡,創新技術被用來理解和概念化醫療情境的問題是什麼,從而讓多元旨趣被促動並徵召,包含臨床、技術、文化、經濟、制度等。本研究主張,創新擴散無法由專業的價值系統所決定,社會團體和技術文化也無法輕易介入,而是複雜且動態的關係本體,由人與非人行動者共同建構,充滿不確定性。
    Transcranial magnetic stimulation (TMS) is an emerging technology that has attracted attention in recent years for its medical use in neuropsychiatric diseases. It has been gradually purchased in hospitals and clinics in different regions and form medical community dedicatedly. This study use Actor-Network Theory (ANT) to analyze the diffusion of TMS, a medical innovation, in Taiwan. From the technology-centered view, we use a qualitative method including secondary data and in-depth interviews to describe the trajectories of TMS in terms of matter of conccern and involved actors in different networks, in order to understand how interests be enacted through the process of translation.

    The study found that (1) TMS moves in different networks, and involed actors are multiple and heterogeneous neuropsychiatric specialties, doing research and therapy, from hospitals and clinics, with scattered or aggregated alliances. (2) The trajectory of TMS is accompanied by the deplacement in matter of concern, from medical problems to clinical issues. When these multiple specialties involve in networks, innovative technology is used to understand and conceptualize what the problems are in medical condition, thus diverse interests, that is clinical, technological, cultural, economic, institutional, are enacted and enrolled in. This study contends that the diffusion of innovation does not solely depend on professional value system, nor can social group or technical culture easily intervene, but is a complex and dynamic relational ontology, jointly constructed by human and non-human actors, thus full of uncertainty.
