    Title: 全民健保醫療品質資訊公開之社會效益評估初探

    Abstract: 為了提升健保醫療品質,衛生署於民國94年成立「全民健康保險醫療品質委員會」,該委員會運作迄今,已獲致初步政策創新,將於近期建立資訊平台,並公開數項重要醫療品質資訊。本研究目的有二:首先,從學術角度分析評估醫療品質委員會指標建構過程,呈現其優勢並檢討其缺失,作為未來政府未來衛生醫療方案規劃過程的參考。第二,對於即將啟用的醫療品質資訊平台運作,以及即將公開的三類疾病醫療品質指標,從醫療提供者、一般民眾與病友的角度進行評估,且由健保局提供的次級資料來分析醫療提供者之行為是否有改變,並提出具體建議,作為未來醫療品質資訊公開的參考,以增強其效益。 為了達到以上目的,本研究採取以下幾種方法:一、檔案分析法:分析該委員會歷次會議過程以及內容。二、深度訪談法:對於「全民健保醫療品質委員會」委員及行政承辦人員進行深度訪談,以進一步了解會議動態過程以及決策模式,此外亦針對三類疾病之醫療提供者進行深度訪談,以了解其對醫療品質資訊公開之意見與可能影響。三、網路問卷調查:針對使用資訊平台之網路使用者進行網路問卷調查,以瞭解其醫療資訊需求以及資訊蒐集管道及行為,以及對目前醫療品質指標與資訊公開平台之看法,以進一步評估其效益。四、焦點團體座談與問卷調查:針對即將公開的三類醫療品質指標之相關病友,舉辦焦點團體座談和問卷調查,以瞭解病友對醫療品質資訊之需求,以及目前規劃之醫療品質指標與資訊公開平台,是否符合其需求或影響其就醫態度與行為。五、次級資料分析法:在醫療品質資訊公開平台運作後,從健保局蒐集醫療提供者之次級資料,進一步分析醫療提供者的醫療行為是否改變。 In 2005, the Ministry of Health founded the Committee of the Quality of Medical Care under the National Health Insurance to promote the quality of medical care under the national health insurance. The committee has hitherto operated successfully to achieve policy innovation by developing several indicators of quality of medical care. By the end of this year, the committee will disclose the information of these indicators through internet. There are two purposes of this research. First, we will analyze and evaluate the decision-making model of the development of these indictors from the academic perspective. Second, we will evaluate the social impacts of the disclosure of these medical-quality indicators from the perspectives of the medical providers, the patients and the general public. As a result of our research, we will provide suggestions for the decision-making model of medical policies and the development of other medical-quality indicators in the future. In order to achieve the above purposes, we will use the following research methods: 1. Document analysis approach: We will review and analyze all documentations of the decision-making process for the development of these indicators. 2. In-depth interview approach: We will have in-depth interviews with the members of the committee and the administrative personnel in order to know the dynamic process for the development of these medial-quality indicators. In addition, we will also have in-depth interviews with medical providers in order to know their opinions toward and the possible impact of the disclosure of these medical-care quality indicators on their treatment behaviors. 3. On-line internet survey: We will conduct an on-line survey of the opinions of the general public who read the information from the internet, in order to evaluate the usefulness of the contents of the information and the way the information is disclosed. 4. Focus-group discussion and questionnaires: We will hold focus-group discussions and conduct questionnaires among the patients of the medical treatment regarding the disclosed medical-quality indicators, in order to know the patients’ opinions toward and the possible impact of the disclosure of the medical-quality information on their decision of the choice of medical providers and services. 5. Secondary data analysis: We will review and analyze the statistics collected from the Bureau of National Health Insurance in order to find out whether there is an immediate change in terms of the treatment behavior of the medical providers.
