    Title: 藥費總額支付制度之可行性評估與建議方案

    Abstract: 全民健保開辦邁向第五年，醫療費用節節成長，財務捉襟見肘，目前已處於入不敷出的虧損期，在不調高保險費費率的前提下，明年初全民健保安全準備即將耗盡，屆時全民健保財務將出現赤字。在藥品費用方面，我國全民健保藥品費用的支出在1996年為622億元，在1997年為640億元（較前一年增加近18億元，年增率為2.88%），在1998年則為722億元（較前一年增加82億餘元，年增率為12.92%），且佔總醫療費用的比例高達26.86%。藥費成長速度，除需分析藥費成長的成因外，有待籌謀對策。 　　我國全民健康保險法中，第五章「醫療費用支付」的第四十七條至第五十條條文，係在規範一般通稱的「總額支付制度」。年度醫療給付費用總額，「得」分地區與分部門（門診及住院）來分配；門診醫療給付費用總額，又得區分出藥品費用總額。然而，全民健保實施四年多來，1998年起方有牙醫門診總額支付制度的試辦；在藥品方面，則長期陷於醫藥分業與藥價黑洞等議題爭辯中，甚少慮及總額支付。 　　國際上實施藥品費用總額支付制度的國家寥寥可數，中外相關文獻亦不多見，實有必要從理論層面出發，探討國外制度與實務經驗，詳加剖析，以其對我國藥費總額支付制度進行可行性評估與提出建議方案。 　　本研究計畫將分為實證面與制度面兩大部份。實證面將以我國全民健保實際資料，分析全民健保藥品費用成長趨勢與結構變化。制度面將探討以下主題：全民健康保險藥品利用現況、藥費成長結構分析；藥費總額支付制度國際經驗探討；.從國內醫藥環境及保險對象用藥品質確保觀點，評估我國實施藥費總額支付制度的可行性；依西醫基層及醫院兩部門分別提出藥費總額支付制度的實施方案，及其相關的配套機制。
