    Title: 全民健保疾病別資料抽樣設計及醫藥費用估算之研究

    Abstract: 各類疾病之醫療費用可提供生病成本或疾病之經濟負擔中直接成本(direct cost)的計算。健保資料中對於投保者雖有各次之就醫紀錄，且紀錄了該次就醫之ICD-9-CM診斷碼，卻未將費用與各診斷碼分離。因此，在討論各類疾病之醫療費用時，以往常將各次就醫之費用全部歸到第一個診斷碼，此即所謂的top-down方法。這種方法的缺點為較常出現在第一診斷碼的疾病其醫療費用會被高估，而較常為非第一診斷碼的疾病其醫療費用會被低估。因此，考慮到疾病間之伴生或共存關係，將醫療費用適當地分配到同時出現的疾病，方可提供各種疾病直接成本之比較基礎。美國的NIH( National Institute of Health)發布了2000年14大死因及46種疾病的直接成本、間接成本、及總成本，加拿大的Health Canada亦公布了1998年18大類疾病的直接成本、間接成本、及總成本。然而，Health Canada的直接成本或間接成本均以主診斷疾病為依歸；NIH雖已將伴生疾病的現象考慮在內，然因人種及環境等之差異，再加上美國並沒有資訊豐富的龐大健保資料庫因而需整合多種調查結果，我們仍需建立合乎我國環境的疾病別醫療費用之估計程序。有鑑於此，行政院衛生署特辦理「全民健保疾病別資料抽樣設計及醫療費用估算之研究」計畫。
