    Title: 全民健康保險藥品市場集中度之研究

    Abstract: 目的： 藥品係一種特殊型式的商品，最終消費者（病人）與生產者（藥廠）或銷售者（藥局）之間無法直接交易；病人需經醫師認可，方能購買處方藥品，藥品的交付需由藥師為之。雖然藥品的異質性相當高，多數情形裡不同藥品可互相替代。醫師固然有決定用藥種類的主導權，不過在台灣的全民健康保險制度內，一半以上的醫師係受雇於醫院，全民健保總藥費中只有五分之一係由基層診所開立。因此，在台灣不完全競爭的藥品市場裡，藥品利用的集中情形值得研究。 研究對像： 國家衛生研究院全民健康保險研究資料庫的健保申報資料。 研究方法： 擬從三方面來分析台灣全民健保藥品市場的集中度情形。在藥品方面，除了一般OLAP (on-line analytical processing) 的分析外，亦將計算DU90%的相關指標。在醫院方面，將利用concentration ratio (CR) 與Hirschman-Herfindahl index (HHI)。在病人方面，則利用Lorenz curve與Gini index。本研究計畫的困難處（也是關鍵處）與創新處，在於將藥品歸類分組（ATC grouping）與轉換計量單位（defined daily dose, DDD），再一一計算測試（decomposition）。此外，在建構完成分析模式後，將考量區域與時間的因素，嘗試找出藥品市場變動的證據。 結論： 探討藥品市場集中度，即可藉以瞭解整個藥品市場。由於藥品市場較為複雜，官方迄今尚無全面詳盡的相關統計正式公佈印行。依學理層層分析所得到的結果，亦將可提供衛生政策與醫務管理參考。
