    摘要: 醫院院史館是一所典藏醫療資源的場所。這些醫療資源包含行政和醫療照護等檔案，協助我們瞭解醫院的運作以及學術研究成果。近世醫學思想日新月異，醫學技術更迭快速，醫院為提供更好的服務，發展腳步之快、層面之廣，實為空前。醫院院史館保存完整的醫療與行政史料，可以呈現醫院人、事、地、物等記錄，提供我們瞭解詳細的醫院資訊。尢其臺灣歷經不同的政治社會背景，我們可以從醫院保存的歷史記錄，瞭解不同政治、經濟、社會、文化等發展的過程，使我們對台灣的過去、現在和未來提供一個不同的視野。
    國外的大型醫院，普遍設有院史館，其管理體制，舉凡組織、人員、經費、館藏、整理和服務上，都有相當的規模，並具備實用的功能。院史館同時也製作網頁，在網際網路上提供資料檢索的服務。國內醫院對於院史館的建置與管理，尚在萌芽階段。為了實際瞭解國內醫院院史館管理體制之發展現況，期能進一步觀察提出醫院院史館建置與管理的原則，乃進行本研究。
    本研究採取問卷調查與訪談法。藉由文獻分析，瞭解國外醫院院史館的組織、人員、經費預算、館藏政策與館藏資料形式、館藏整理和所提供服務等項，將這些資料與臺灣醫院院史館管理制度現況對照研究。
    研究結論為：（1）成立院史館：64所醫院只有13所醫院成立院史館，而13所成立院史館以宗教醫院數量最多，有6所；其次為榮民醫院4所；軍醫院2所和學校附設醫院1所；（2）隸屬單位：64所醫院院史保存單位，50％隸屬於院長室和秘書室之下；（3）組織地位：64所醫院院史保存單位組織地位，36％為一級單位，16％二級單位，3％三級單位；（4）編列預算：64所院史保存單位，只有13所醫院編列預算，佔全部20 . 3 %又多是採取實報實銷方式；（5）人力資源：64所醫院院史保存人員，多為兼職性工作；人數多為一人，有二人至三人則各有1所；教育學識背景具有圖書館學系有1所，歷史學系/所有2所，其他為非關學系；（6）館藏政策：64所醫院只有20 . 3％有擬訂館藏政策；（7）館藏資料形式：64所醫院70 . 3％都有保存照片，其次為出版品、獎牌/狀、歷屆院長、剪輯資料和視聽資料；（8）館藏資料整理：64所醫院 12 . 5％的館藏資料，依照年代分類，其次為部門、形式和主題排列；25 . 7％醫院以Word文書作業軟體作為館藏編排與建檔；64所醫院能提供書目查詢只有9所，提供資料庫查詢有1所；（9）院史館管理辦法：64所醫院只有10所成立院史館的醫院擬訂「院史館管理辦法」；（10）展覽室：64所醫院只有11所設置展覽室；（11）提供服務：64所醫院有成立院史館的醫院，以展覽為主要服務項目，其次為導覽、參考服務和影印等服務。
    綜合研究結果，顯示臺灣醫院院史館之管理體制，仍處於發展階段尚未制度化，冀望借鏡於國外例子，助益國內醫院院史館管理體制的發展更臻完善。
