| Abstract: | 「憂鬱症」已成為一種全球性的現象，所謂的「憂鬱世代」論述更是大行其道，然而，在本研究中，我希望能深入探討這些「認為自己有憂鬱症，卻被排除在診斷之外」的人們，他們如何、又為何感受到憂鬱情緒所帶來的「反常」困擾？為此，本文參考了情緒史、病識感研究、比較文化研究及醫療社會學研究的資料，受到傅柯系譜學研究的啟發，試圖以「憂鬱症病識感」作為主要的觀察對象，理解「憂鬱情緒」如何受社會影響，進而影響世代的自我認知。
本研究透過整理台灣精神醫療現代化的40多年進程，釐清「官方」政策的疾病認知以及精神疾病防治推廣的論述如何轉變，並嘗試證明這些論述如何相互交織，形成現行民眾對於精神醫療的整體理解。此後，本文透過半結構式訪談，建構出現況下的「憂鬱世代」樣貌，嘗試回應前兩章的內容，透過精神科醫師在醫療臨床現場的觀察，及醫療使用者的自我敘事，進而驗證官方政策的認知轉向與情緒覺察論述的建立，如何影響醫療使用者對「憂鬱情緒」的界定，進而產生「憂鬱症病識感」。
透過前述章節的整理，本文爬梳了台灣精神醫療40多年來的歷程，對憂鬱症防治政策進行盤整與反思，還原出「憂鬱情緒」在台灣脈絡下的展演與再現，並試著回應傅柯的觀點，藉由實際訪談處於「病」與「非病」之間的醫療使用者，嘗試提出一種有別於「規訓社會」的社會想像，建立一種「情緒解放」的可能性。另外，本文也以「世代」的觀點回應傅柯的歷史本體論中屬於「時代」的假設，提出行動者在同一時代下可能呈現的異質性，並回溯到不同醫療使用者的生命歷程，嘗試找出社會中的情緒覺察機制如何促進「自我」對於「憂鬱情緒」的理解與建構。
"Depression" has become a global phenomenon, and the discourse of "Depressed Generation" has become more popular. However, in this thesis, I hope to explore people who believes themselves suffer from depression, but be excluded from diagnosis. We want to figure out how and why do they feel the “abnormal” troubles caused by melancholy? For this reason, this thesis refers to the research of emotional history, disease perception, comparative cultural study, and medical sociology, then inspired by Michel Foucault’s theory of genealogy. We are trying to use "the insight of depression " as the main observation object, to understand how melancholy is affected by society, which in turn affects the self-perception of generations.
By sorting out the 40-year process of psychiatric modernization in Taiwan, this study clarifies how the "official" policy perception of disease and the discourse on the promotion of mental illness prevention and treatment policies have changed, and try to demonstrate how these discourses are intertwined to form the current public`s understanding of psychiatric medicine. After that, this thesis constructs the appearance of the "Depressed Generation" under the current situation through semi-structured interviews, and attempts to respond to the content of the first two chapters. Through the observation of psychiatrists in the medical clinical scene and the self-narratives of medical users, it is further verified. How does the cognitive turn of official policy and the establishment of emotional awareness discourse affect medical users` definition of melancholy, and then produce "the insight of depression ".
Through the organization of the preceding chapters, this thesis has reviewed the history of Taiwan`s psychiatric medical care for more than 40 years, restructured and reflected on the depression prevention and treatment policies, restored the performance and reproduction of melancholy in the context of Taiwan, and tried to respond to Michel Foucault. Through actual interviews with medical users who are between "disease" and "non-disease", try to propose a social imagination different from "disciplinary society" and establish a possibility of "emotional liberation". In addition, this thesis also responds to the assumption of “epoch” in Foucault’s historical ontology from the perspective of “generation”. We want to propose the heterogeneity that actors may present in the same era, and goes back to the life course of different medical users. Find out how the emotional perception mechanism in society promotes the individual understanding and construction of "depressed emotions." |
