    摘要: 此刻正積極規劃與推動的長期照顧保險被譽為台灣建構完善福利國家的最後一項工程。但對位處偏鄉的原住民而言,缺乏文化能力的社會福利等同扼殺原住民靈魂的文化殖民手段,長期照顧在制度的設計上如何在「普遍平等」的公平原則中,保有「尊重差異」的多元彈性空間,成為長照制度設計的重大考驗。接續本人目前進行的「居家服務在原鄉」研究計畫,以屏東縣介惠基金會在八個原鄉的服務經驗進行整理,發現介惠在原鄉居家服務的在地化策略主要是多數外來承辦組織採取的策略,也就是以「聘僱在地人」的方式進行;在這個初步研究發現的基礎上,在探詢「以部落為中心」長期照顧服務精神的在地實踐,我希望以在推動居家服務方案的過程中,不僅聘僱在地人,還在組織層面上,與在地組織發展合作關係的外來組織為研究對象,希望瞭解外來組織與部落在地組織之間在居家服務推動的過程中如何發展出尊重部落意願的服務提供策略。因此,我將以在台東縣原鄉推動居家護理三十年的聖母醫院為研究對象,期望透過記錄與分析聖母醫院在原鄉推動居家服務的歷程,以及過程中聖母醫院與部落之間的關係,以提供現有與未來將在原鄉提供居家服務的外來組織想像如何與部落一起工作發展符合在地文化的居家服務。本研究為三年期的研究,將以台東聖母醫院提供居家服務的原鄉為研究對象,運用建制民族誌的設計,以部落為立足點,分析聖母醫院與部落在地組織如何在現有居家服務政策的框架中,走出符合部落主體性的照顧服務。我將選擇阿美族的泰源部落、排灣族的嘉蘭部落、以及達悟族的蘭嶼鄉為研究場域,紀錄部落實作居家照顧服務的經驗。台東聖母醫院與部落合作的經驗將讓我們看到現有居家服務體制背後「原住民文化盲目」的政策預設,但也看到聖母醫院如何使部落意願可以展現在居家服務的提供,從台東聖母醫院的經驗我們將試圖分析作為目前正在積極規劃的長期照顧政策朝向多元文化精神的政策參考。
    Long term care insurance which is under process is seen as the last piece of a comprehensive welfare state for Taiwan. However, a welfare policy which is blind to indigenous cultures can be a means of cultural genocide. For indigenous peoples in the reserve areas, the implementation of long term care insurance could mean that the services they receive deny their traditional ways of life, if cultural variation is not taken into consideration. How to design a long term care system which integrates the principles of equality for all and respect for diversity is a challenge. In my current research project, I have documented the home care experience of Jieh Huey Social Welfare & Charity Foundation in the indigenous communities of Pin-Tong County. Jieh Huey’s relationship with indigenous communities is to hire indigenous people as home care workers on the individual base. Jieh Huey did not develop inter-organizational relationship with any local indigenous organization. In the proposed three-year research project, I intended to focus on home care agencies which choose to work with the communities in the delivery of home care services. This research will focus on the St. Mary Hospital in Tai Tung County, which has started its home health nurse care since 1982. Three communities, which have developed various cooperative relationship with the St. Mary Hospital, will be chosen as the study sites, which include the Tai-Yuan community of Amis tribe, Chia-Lan community of Piwan tribe, and the Orchid Island of Tao tribe. The research is aimed to identify the gaps between the home care policy and the lived world of indigenous peoples with a vision to develop an inter-organizational structure to integrate local culture into the design and delivery of long term care system. The research adopts institutional ethnography as research design to investing how indigenous communities develop home care services that reflect their ways of living. These instances of resistance offer a way to imagine alternative ways of thinking about home care in indigenous communities.
