    摘要: 精神障礙者之家庭擔負著重大且長久之照顧責任,呈現多重挑戰和需求,因此照顧負荷成為過去經常被研究的主題,也是此領域之家庭介入的焦點之一。然其乃立基於缺失模式,嘗試彌補不足,且焦點仍在藉由對於照顧者之協助間接促進精障者治療和復健之成效。然而近二十年,對於精障者家庭之觀點有了巨大變革,採取較平衡之視角,除了照顧負荷之外,亦能看見其優勢和復原力,家庭介入的理念和取向亦由病態模式轉為能力模式和成長-發展模式,並且提出對於家庭服務應該含括照顧者之福祉和健康為指標。西方對於精障者家庭介入仍以病理或缺失模式居多,以優勢觀點進行介入且加以評估者則尚未見。筆者目前進行之國科會計畫針對精障者實施優勢觀點個案管理模式一年餘,儘管已見初步成效,其間亦觀察到家庭成員之態度和關係可發揮載舟或覆舟之影響。因此預計延伸既有處遇和研究對象至精障者之家庭成員,進行系統性且個別化之家庭處遇,同時促進精障者和照顧者之復元。本研究將兼採量化和質化取向,以準實驗設計,於兩年半內進行六次的施測,探索優點個管模式家庭處遇對於二者之復元結果的影響,以及其影響因素;再者,亦將分析精障者和照顧者兩造之復元結果之相互關係。服務方案之實施對象為高雄長庚醫院日間留院和呈泰復健中心之學員和其家庭成員,共120 個家庭。資料搜集對象包括精障者和其主要照顧者。質性深度訪談則探究研究參與者之改變(復元)歷程,以及優點個管模式對其影響。本研究可回應精障者家庭之需求,同時具有驗證理論之意涵。
    The families of persons with psychiatric disability have shouldered tremendous caregiving responsibilities and faced challenges in many aspects. The family interventions in the West has been based on pathological (deficit) model and tried to provide remedies for the caregiver burden and indirectly facilitate the treatment outcomes of persons with psychiatric disability. However, the literature revealed that there has been a pronounced paradigm shift in professional practice with families, from a model of pathology to one of competence and growth-development. In addition, nowadays professionals hold a more balanced view of the caregivers in that they not only experience burden but also demonstrate strengths and resilience. The author has conducted a two-year experiment on persons with psychiatric disability in two rehabilitation facilities and the preliminary data has shown positive outcomes. Nevertheless, there are challenges in promoting recovery and the attitude and response of family members is one of them. Thus, the author proposes a strengths based family intervention program, and aims to facilitate the recovery for both the persons with psychiatric disability and the family members, especially caregivers. The study will adopted a mixed design with both quantitative and qualitative approaches. By using a quasi-experimental design, the study will follow the changes of the recovery status on 120 pairs of persons of psychiatric disability and their caregivers. The study will examine the effects of strengths based family intervention on the recovery of both types of participants and the correlation of the recovery status between the pairs. The qualitative in-depth interviews will be conducted to explore the recovery process of the pairs and how the strengths based family intervention affect their recovery. This study will have implication for the practice because it responds to the needs of family. In addition, it will have strong implications on treatment theory and theoretical model of recovery.
