    摘要: 本研究旨在探討慢性偏頭痛與多向度睡眠健康之間的雙向關聯性。過去研究指出,睡眠障礙與偏頭痛症狀間具有密切的雙向互動,例如睡眠不足或睡眠品質不佳可能增加偏頭痛的頻率與嚴重程度,而偏頭痛發作也會反過來影響睡眠狀態,如造成入睡困難、睡眠中斷或早醒等。然而,現有文獻多以橫斷式研究為主,特別是針對慢性偏頭痛患者為對象的研究,更缺乏每日睡眠與頭痛症狀間雙向關係的實證資料。因此,本研究透過14天的主、客觀並用睡眠記錄,並且以生態瞬時評估重複測量慢性偏頭痛患者的頭痛症狀,以檢視當日睡眠健康對隔日偏頭痛症狀的預測效果,以及當日偏頭痛症狀對當日睡眠健康的影響。
    研究對21~60歲的慢性偏頭痛患者,包含4名男性,19名女性。參與記錄前,經半結構及自陳問卷初步篩檢可能的精神疾病及睡眠障礙風險。其後,持續配戴腕錶式睡眠活動紀錄儀十四天,並每日以手機進行三次生態瞬時評估,記錄頭痛次數、發生時間、維持時間、最大強度、止痛藥使用情形,以及睡眠日誌,記錄完成後,受試者填寫RU-SATED問卷 、匹茲堡睡眠品質量表、貝克憂鬱量表、貝克焦慮量表。其結果以階層線性模型進行分析,模型主要變項包括:主、客觀睡眠健康分數、RU-SATED分數、頭痛症狀 ( 頭痛次數、維持時間、最大強度 );控制變項則為用藥情形 ( 睡眠藥物、急性止痛藥、頭痛預防藥 )、憂鬱 ( 貝克憂鬱量表分數 )、焦慮 ( 貝克焦慮量表分數 ) 與整體自陳睡眠品質 ( 匹茲堡睡眠品質量表分數 )。
    研究結果顯示,兩週記錄之睡眠健康分數對頭痛症狀具有顯著的負向預測力,即睡眠健康愈好,頭痛症狀愈輕;然而,個體內每日層次的睡眠對於頭痛關聯則未達顯著,顯示其影響可能較長期而非每日立即顯現。此外,當日頭痛最大強度對當日的睡眠健康有顯著負向關聯,即當日頭痛最大強度會影響當日睡眠健康。
    綜合而言,本研究發現長期睡眠健康能預測慢性偏頭痛症狀表現,頭痛最中大強度亦會影響當日睡眠健康,兩者雙向互相影響。此結果凸顯睡眠管理對慢性偏頭痛患者的重要性。未來研究建議可進一步探討長期累積效果與其他可能影響因素,以更深入理解睡眠與偏頭痛之間複雜的交互關係。
    This study investigates the bidirectional relationship between chronic migraine and multidimensional sleep health. Prior research has indicated a strong bidirectional interaction between sleep disturbances and migraine symptoms, whereby insufficient sleep or poor sleep quality may increase migraine frequency and severity, while migraine attacks can, in turn, disrupt sleep, leading to difficulties initiating sleep, sleep interruptions, or early awakenings. However, the majority of existing studies are cross-sectional in design. Notably, empirical research focusing on chronic migraine patients and examining daily bidirectional dynamics between sleep and headache symptoms remains particularly limited. Thus, this study employed a 14-day protocol combining subjective and objective sleep monitoring, along with ecological momentary assessment (EMA) to repeatedly measure headache symptoms in chronic migraine patients, to evaluate the predictive effect of daily sleep health on next-day migraine symptoms and the impact of daily migraine symptoms on same-day sleep health.
    Participants were chronic migraine patients aged 21–60, including 4 males and 19 females. Before the study, participants underwent screening for potential psychiatric disorders and sleep disturbance risks via semi-structured interviews and self-report questionnaires. Subsequently, they wore wrist actigraphy for 14 days and completed EMA three times daily via smartphone to record headache frequency, onset time, duration, intensity, analgesic use, as well as a sleep diary. After the 14-day period, participants completed the RU-SATED sleep health scale (RU-SATED) , Pittsburgh Sleep Quality Index (PSQI), Beck Depression Inventory (BDI-II), and Beck Anxiety Inventory (BAI). Data were analyzed using hierarchical linear modeling, with primary variables encompassing subjective and objective sleep parameters, RU-SATED scores and headache symptoms (frequency, duration, intensity). Control variables included medication use, depressive levels (BDI-II scores), anxiety levels (BAI scores), and self-reported sleep quality (PSQI scores).
    The findings revealed that sleep health over the two-week period had a significant negative predictive effect on headache symptoms, indicating that better sleep quality was associated with reduced migraine symptoms. However, intraindividual daily-level associations were not significant, suggesting that bidirectional influences between sleep and migraines may emerge over longer timeframes rather than daily. Additionally, same-day headache intensity showed a significant negative association with same-day sleep health, indicating that greater headache intensity on a given day adversely affects sleep health on that day.
    In conclusion, this study demonstrates that long-term sleep health can predict the manifestation of chronic migraine symptoms, while daily headache intensity can also impact same-day sleep health, suggesting a bidirectional relationship between the two. These findings highlight the importance of sleep management in patients with chronic migraine. Future research should further investigate long-term cumulative effects and other potential influencing factors to better understand the complex interplay between sleep and migraines.
