    題名: 全民健康保險總額制度執行架構之評估

    摘要: 【動機與目的】 健保總額支付制度是一種預先以協商的方式訂定未來醫療服務總支出的預算制度，其目的主要是對財務容易失控的社會保險進行成本控制。台灣的制度就行政執行面觀之，其運作的核心除了政府的行政部門以外，主要是由衛生署層級的費用協定委員會、健保局層級的全民健康保險總額支付委員會、以及健保局各區分局的執行委員會為其平台，這是一種標準的「跨部門協力」（multisector collaberation）之公共治理（public governance）模式，對於健保總額制度的執行績效，取決於這個公共治理模式運作的良窳。近年來人民團體蓬勃發展，參與政策之意願及能力均較過去提高，而各人民團體間規模、屬性差異甚大，或有會員重疊，政策利害關係人難以認定與參與代表性的質疑，造成各個不同類別的總額支付委員會的對醫療費用支出的控制能力有所不同。有鑑於此，本研究希冀透過此研究綜整上述委員會所形成的管理網絡進行系統性探索與評估。基此，主要研究目的羅列如下： 1. 各總額執行委員會及分區執行委員會之組成與運作方式現況探討 2. 各總額支付委員會及執行委員會之運作方式的比較，以及優缺點分析。 3. 總額支付制度運作模式的建議。 【研究設計】 為達到前述研究目的，本研究將採取下述五種研究方法進行資料蒐集：1. 文獻蒐集--了解相關法規暨建構本文評估架構；2. 深度訪談--了解總額支付委員會與執委會運作現況；3. 參與觀察--實際參與會議進行，體會驗證質、量方法之發現；4. 面訪調查—訪問各支付委員會委員，評估各總額支付委員會的運作機制；5. 電話訪問調查--探究各分局與總額部門的聯繫方式。 【結論與建議】 研究的結果顯示，中央健保局與各總額的醫事團體在結構設計以及政治兩項有待改善，若就四個總額部門進行比較來看，中醫總額的全聯會的組織運作較為完備，而牙醫總額全聯會的組織運作次之，醫院總額的全聯會則相對有改善的空間，至於公私協力部分，健保局人員與醫事團體人員就委員會的目標共識上有待加強。政策建議主要可區分為兩部份，一為針對後續總額支付委員會籌組的建議，主要包括：維持委員多方代表結構、建立資訊公開與退場機制、以及強化學者專家的代表性等三項；另一為針對後續總額支付執行架構在組織內部(健保局、全聯會)以及組織間(健保局與全聯會之間的公私協力)的運作建議，包括健保局內部組織運作的調整、公私協力夥伴參與者的共識建立、協助醫事團體自主管理、清楚界定委員會權力與義務、和給予委員專業人力的支援等五項。 [Research proposes] There are four proposes in this project: 1. An analysis of the implementation mechanism of the Global Budget Payment Committees in the National Health Insurance Bureau. 2. An analysis of the implementation mechanism of Global Budget Implementation Committees and Regional Global Budget Implementation Committees. 3. A comparative analysis of the different Global Budget Payment Committees and Global Budget Implementation Committees. 4. Providing policy recommendations for improvement of the implementation mechanism of the Global Budget Payment System. [Methodology] The methods used in this research are literature reviews, questionnaires, face-to-face interviews, participatory observation. First hand and secondary data are collected to understand the institutional background and bring up questions to be discussed in this research. The data from questionnaire are analyzed through the SPSS. [Results] As recommend by the results of this project, the global budget payment system can be improved through the following ways: reforming the internal organizational structure of the BNHI, building consensus between public and private sectors, assisting providers’ self-governing activities, clearly defining the responsibility of members in the Global Budget Payment Committee, and providing relative resources to support the works of the Global Budget Payment Committee.
