    Title: 從民眾觀點建構健保評量指標第二年研究

    Abstract: 本研究將根據第一年的研究發現與面對的研究限制，繼續從廣義的「民眾」觀點(包括「關注民眾」觀點與「一般民眾」觀點)建構可操作化、可測量與可管理的健保政策評量指標與評量機制，以提升民眾對健保的政策理性與支持度，並作為日後提升醫療品質、服務品質、調整保費和其他健保改革的重要參考。 為達成上述目標，本研究主要工作項目與方法如下： 1.文獻與相關資料的檢視與分析：由於第一年研究提出的構面與其指標仍有不清晰處，本計畫第二年持續對於健保政策之評量指標進行相關文獻資料的蒐集，以及進行國際經驗比較的資料蒐集。 2.焦點團體座談法：本計畫第二年擬針對參與第一年政策德菲法之專家學者、醫、藥、勞、資、社會等團體代表為邀請基礎，舉辦焦點團體座談會，以深入討論健保政策評量指標，並作為第二年政策德菲法與一般民眾調查項目之依據。 3.訪談法：對於「關注民眾」、「一般民眾」有共識健保政策評量指標的資料與測量來源，與目前健保統計資料庫之內容，進行衛生署與健保局資料庫相關人員的訪談，以作為提出可操作、可測量與可管理指標之根據。 4.政策德菲法：本研究擬邀集健保政策的「關注民眾」進行二個回合的腦力激盪與「紙上會議」對話，將修正過的構面、指標與操作性定義進行調查，作為電話調查的問題依據。 5.電話調查法：以全國二十歲以上民眾為母體，電話調查全國1,068位以上之民眾，詢問其對於健保政策評量指標、健保業務與財務監理指標、健保的自費議題、健保知能、資訊揭露內容與方式等相關問題的意見。 6.根據以上研究發現，撰寫期末報告，針對從廣義民眾觀點建構操作化、可測量與可管理的健保評估指標與資訊揭露機制，提出具體建議。 The implementation of Taiwan National Health Insurance has been for more than 10 years. The government has committed to maintaining the world`s advanced countries in comparison to the low health insurance rates. Also, the medical costs account for the gross national product (GDP) and the proportion is significantly lower than the world`s advanced countries. In addition, the British Economist magazine rated medical services indicators of National Health Insurance as the second best one in the world. Also, Bureau of National Health Insurance has commissioned several studies to prove medical quality of National Health Insurance to be better than many advanced countries. The public opinion survey found that 80 percent of people were satisfied with the program. However, the government faced strong opposition from the general public in attempting to raise rates. This study will be a broad "public" point of view (including "policy elites" views and "the general public" viewpoint) to evaluate the policy performance and present evaluation indicators. The survey methods include secondary data collection and literature review to analyze the relevant policy evaluation theory and case-related indicators as the theoretical framework of this study. Also, the research applies policy Delphi method, telephone survey, and focus group discussion to empirical study. Finally, the research will present research findings and policy suggestions about evaluation indicators.
