    Title: 台灣長期照護財務制度建立之研究

    Abstract: 長期照護問題是一連續且綜合性的問題，包括社會支持、保健、醫 療、復健等及其所對應之財務體系。由於長期照護體系的建構需要投入 相當財源，因此常使決策階層裹足不前，尤其在國家財政拮据時期，更 擔心其會影響國家財政，不敢貿然承諾。因此長期照護財務體系的規劃， 必需把握原則與方向，以降低其對我國財政衝擊的憂慮。有關台灣長期 照護財務現況之檢討，本研究將從兩個面向切入觀察：其一是財務責任 釐清與定位；其二是財務資源之有效配置。目前我國長期照護的財源以 及相關服務等分散於不同之行政與服務體系，尚未建置綜合性及連續性 之長期照護財務及行政體系。體系的紛歧，往往使得需要醫療照護或生 活照顧的老人及其家庭不知利用；且體系間各自發展，不但造成資源重 複投資，亦容易形成發展不均與服務斷層等現象。目前我國長期照護的 主要財源，仍以家庭為主，全民健康保險僅針對部份慢性病人及需醫療 性質居家照護之病人提供部份給付。社會福利對長期照護之病人，則多 以低收入戶為主要的服務及補助對象。由於我國目前長期照護尚缺乏綜 合性及連續性且制度化之財務體系，將來實有待政府社政與衛生單位共 同研擬，並作整體性之前瞻規劃。
