    摘要: 無論國內外,罹患癌症的機率隨著年齡的增長而逐漸上升。若罹癌者為家庭主要經濟支柱,其帶來的衝擊伴隨著情緒、經濟、照顧等影響家庭功能的可能。因此,本研究旨在瞭解罹癌家庭在過程中的主體經驗與挑戰,期望藉由家庭為單位瞭解經歷癌症不同階段的調適歷程,探討罹癌家庭中的社會支持,並在最後討論癌症為家庭成員帶來的意義。
    本研究採用質性取向,以立意取樣與滾雪球方法找尋研究參與者,並使用深度訪談法,輔以半結構式訪談大綱探詢罹癌家庭中不同成員間的主觀感受。最後以主題分析方式分析訪談內容進而捕捉罹癌家庭的調適經驗。研究對象為三戶家庭,內含:三位罹癌者、三位照顧者以及六位子女,共計十二位研究參與者。
    透過家庭調適模型中對於家庭需求、家庭能力與家庭意義的討論,本研究發現自我暗示、自我充權、社會比較、原生家庭、轉移注意力以及影響子女未來職涯選擇,調適癌症所帶來的家庭需求與挑戰等,致使罹癌家庭的社會支持與其家庭調適有不同層面的影響。
    對於研究建議涵蓋三個部分,首先,政策建議包含:完善家庭外部支持體系、減輕罹癌家庭的經濟與照顧議題;其次,實務建議包含:協助維持家庭平衡與跨網絡合作;最後,對於未來研究建議則可以針對不同癌症類別或不同家庭類型進行更深度的討論,以豐富罹癌家庭的研究。
    When the individual diagnosed with cancer is the primary economic provider in the household, the diagnosis can bring significant emotional, financial, and caregiving challenges that may affect overall family functioning. This study aims to explore the subjective experiences and challenges faced by cancer-stricken families. By examining families as units, the research seeks to understand how families adjust and adapt across different stages of the cancer journey, investigate the role of social support within these families, and explore the meaning cancer brings to family members.
    This research adopts a qualitative approach. Participants were recruited through purposive and snowball sampling methods, and data were collected via in-depth interviews using a semi-structured interview guide to explore the subjective experiences of various family members in cancer-stricken households. A thematic analysis was conducted to interpret the interview data and capture the experiences of families dealing with cancer. The participants included three individuals diagnosed with cancer, three caregivers, and six children, representing a total of twelve participants from three families.
    Through the lens of the Family Adjustment and Adaptation Response (FAAR) model, which addresses family demands, capabilities, and meanings, the study found that autosuggestion, self-empowerment, social comparison, family-of-origin experiences, distraction techniques, and influence on children’s future career choices were among the strategies used to meet the challenges of cancer. These coping mechanisms significantly influenced both social support systems and family adjustment and adaptation at various levels.
    The study purposes following recommendations. First, in terms of policy: it advocates for strengthening external support systems for families and alleviating the financial burdens of cancer-stricken families. Second, for practice: it suggests maintaining family balance and promoting inter-agency and cross-network collaboration. Finally, for future research: it recommends further exploration into different types of cancer or diverse family structures to enrich the understanding of cancer-stricken families.
