    Title: 自閉症兒童社交溝通能力之家長遠距訓練方案療效研究:單一個案研究法
    Telehealth-based Parent-mediated Intervention Program for Enhancing Social Communication Skills in Children with Autism Spectrum Disorder: Single-case Study

    Abstract: 研究目的:本研究以自然且具發展性的行為介入取向為藍本,藉由遠距視訊家長執行的方法,設計以社交溝通為主的課程。對自閉症類群障礙兒童(Autism Spectrum Disorder, ASD)之家長進行介入課程,並探討此課程對於家長及兒童的成效。
    研究方法:本研究採單一個案研究法納入3名ASD男童(49個月、35個月和52個月),依照跨參與者間的設計法依序納入3名參與者進行遠距介入。每位家長進行1週1次,每次1小時,連續12週的介入課程。主要療效指標有三項:(1)以自然且具發展的行為介入真確度檢核為評量家長執行療育技巧的頻次及品質;(2)紀錄兒童自發溝通次數;(3)紀錄兒童共享式參與的比例。療效指標以家長拍攝之親子互動影片分析,並將資料分成基線期、介入期和追蹤期。
    研究結果:家長介入真確度方面,3名家長介入期與基線期資料相比不重疊率(PND)指標顯示有中度或高度的效果,效果量達大效果量。在介入撤除後一個月之追蹤期,表現大致可維持。兒童自發功能性溝通次數方面,3名兒童介入期與基線期資料相比不重疊率(PND)指標顯示有中度的介入效果,效果量屬小效果量。3名兒童的能力大致可維持至追蹤期。兒童共享式參與方面,3名兒童在介入期與基線期資料相比不重疊率(PND)指標顯示有中度至高度的效果,效果量屬小效果量,能力大致可維持至追蹤期。不過,單就共享式參與中的協調一致共享式參與表現而言,不重疊率(PND)指標顯示有低度至中度的介入效果,效果量達大效果量,至追蹤期時能力尚可維持。
    研究結論與建議:採用個案研究法實施遠距視訊家長執行介入課程,可提升家長真確度表現與兒童溝通次數和共享式參與表現。不過本研究僅納入3名自閉症兒童及其家長作為參與者,未來宜擴大樣本,採取組間設計,以釐清諸多療效因子。
    Purpose: This study used a naturalistic developmental behavioral intervention approach as the framework, designing a curriculum focused on social communication through telehealth-based parent-mediated intervention methods. The aimed to examine effectiveness outcomes on parents and children.
    Method: Multiple baseline design of single case study recruited three ASD boys (aged 49 months, 35 months and 52 months) and training their parents in sequence mediated by telehealth. The study arranged 12-week, one-hour, once-a-week program for parents of children with autism spectrum disorder (ASD). The main efficacy indicators are three: (1) naturalistic developmental behavioral interventions fidelity examine the frequency and quality of parents’ intervention skills; (2) coding the frequency of spontaneous functional communication; (3) coding and calculating the proportion of joint engagement. All of the indicators are analyzed by the parent-child interaction videos which taken by the parents at home. Those data were divided into baseline, intervention and follow-up periods.
    Results: The study showed that for parent intervention fidelity, the percentage of non-overlapping data (PND) showed moderate to high intervention effect between the intervention period and the baseline period data, and the effect size was high. After 1-month follow up, the performance was generally maintained. For the frequency of child`s spontaneous functional communication, the PND showed moderate intervention effect, but the effect size was low. The ability of the three children were maintained until the 1-month follow up. For child`s joint engagement proportion, the PND between the intervention period and the baseline period data showed moderate to high intervention effects, but the effect size was low. When analyzed coordinated joint engagement independently, the PND showed moderate intervention effect, and the effect size was high. The ability could maintain until the 1-month follow up.
    Conclusion and Recommendation: The single case study method was employed using telehealth parent-mediated training program with single case study could improve parent’s intervention fidelity, ASD child’ s spontaneous communication and joint engagement status. However, the study only included 3 children and their parents as participants. In the future, it is recommended to expand the sample size and adopt between-group design to clarify various intervention factors.
