    Title: 初診斷乳癌患者不同因應型態與心理適應的關係之探討
    Exploration of the Relationship between Different Coping Styles and Psychological Adaptation in First-diagnosed Breast Cancer Patients

    Abstract: 研究目的:本研究主要目的在探討乳癌患者的使用之不同因應類型與心理適應間的關係。過去討論乳癌患者的因應時,多聚焦於特定因應策略對心理適應的影響,但較忽略了個體可能同時使用不同類型的因應策略。究竟趨近式因應與逃避式因應對乳癌患者的心理適應之影響為何,其交互作用又會帶來怎麼樣的影響,於過去,是較少被探討的。此為本研究的目的。
    研究方法:本研究採縱貫研究法,透過立意取樣以269名中部某教學醫院之初診斷乳癌患者為對象,進行兩年期的追蹤研究,追蹤時間點為術後一年(T1)、術後兩年(T2),並蒐集與測量個案的基本資料、因應策略與焦慮憂鬱分數。本研究以使用頻率之中位數將趨近式因應與逃避式因應各自區分為多與少兩組。統計部份,採混合設計三因子共變異數分析,探討不同因應類別與時間及其交互作用對焦慮及憂鬱情緒的影響效果。最後以階層迴歸分析檢驗趨近式因應、逃避式因應及其交互作用對焦慮與憂鬱分數的預測關係,與共變異數分析之結果進行交叉驗證。
    研究結果:在共變異數分析結果上,趨近式因應、逃避式因應及「趨近式因應*逃避式因應」的交互作用都對乳癌患者的焦慮及憂鬱有影響效果,時間對焦慮有主要效果。在階層迴歸分析的結果方面,T1的趨近式因應與逃避式因應能分別預測T1焦慮與憂鬱,且兩者間具交互作用關係,趨近式因應能調節逃避式因應對T1焦慮與憂鬱的影響;另外,T1的趨近式因應與逃避式因應能分別預測T2焦慮與憂鬱,在焦慮情緒上,兩者具有交互作用效果,趨近式因應調節了逃避式因應對於T2焦慮情緒的影響,但在憂鬱情緒上,兩者並無顯著的交互作用關係。
    結論:趨近式因應策略的使用會降低乳癌患者的焦慮及憂鬱症狀,而逃避性因應策略的使用,會與乳癌患者較多的焦慮及憂鬱情緒有關,但兩種策略的使用效果會受到彼此的影響。趨近式因應的使用可補償逃避式因應對患者的負面影響,然而逃避式因應亦會減弱趨近式因應對患者的正面影響效果。乳癌患者應減少逃避性因應策略的使用、增加趨近式因應策略的使用,方能邁向更好的心理適應結果。
