Title:	從政策網絡析論我國器官勸募網絡實務運作 An Analysis of the Practical Operation of the Network of Organ Procurement Organizations from Policy Network

Abstract:	我國推動器官勸募網絡超過十年有餘，每年藉由10餘家勸募醫院，採取結盟關係型態串連起全臺數百家合作醫院，共同執行器官勸募業務的推動工作，然而2011年8月愛滋器官誤植事件後，凸顯制度上及互動上的問題，遂導致政策變遷。爰此，本研究以此關鍵轉折時間，運用政策網絡隱喻性的動態關係，來分析我國器官勸募網絡之制度推展的理念背景、器官誤植事件後影響制度變遷的內容，制度調整前後，對於主要行動者（勸募醫院）所帶來的互動關係影響。本研究採取深度訪談法及資料分析式，主要聚焦北部器官勸募網絡組織，以此來探討器官勸募網絡實際動態運作面貌，並從中發掘本研究對我國器官勸募網絡所作的特性觀察。研究後得出以下結論：在制度調整前，因無良好的互信及互動，複又引入競爭機制，遂導致以爭奪合作醫院為主等非預期性副作用情形產生；在制度調整後，因器官誘因被切割出來，勸募醫院與較具資源或實力的中大型合作醫院，又在資源依賴連結低的關係下，互動結構從上下「強連結」轉變為「弱連結」關係；然而整體網絡調整趨勢中，可觀察到已逐步由競爭式轉趨為合作式的導向；而在制度調整過程中，政府以強制力量來引導並將規範逐步收斂嚴謹，顯見政府為網絡中必要行動者，政策權威與法令仍是一股重要力量；這些制度上的調整，讓勸募醫院行動者落實推廣勸募業務的實務作為，有逐步達成行動者「信念」與「實務作為」趨於一致情況；惟本研究預測，調整後的新制度，北部區域因少數勸募醫院掌控權力及資源，恐進一步衍生網絡權力較勁與階層化。最後，本研究建議除了透過網路行動者間的目標與價值整合之外，政府亦必須提升網絡治理能力，並建立網絡協力合作的責信評量制度，促使網絡行動者互動關係強化、和諧與平衡的發展。 The organ procurement network in Taiwan has been developing for more than 10 years. The network is composed of over 10 hospitals as organ procurement organizations (OPOs) and they develop coalitions with other hundreds of hospitals across Taiwan to promote and execute the organ procurement program. However, the incident of mis-transplant in August 2011 that exposed the operational problem of the network led to an institutional change. Focusing on this critical event, this study analyzes the institutional background of the organ procurement network, the policy change after the incident and the impact on the interaction between the major actors (i.e. the OPO) before and after the change by examining the metaphorical dynamic of the policy network. This study adopts the methods of in-depth interview and documentary analysis, and focuses on the network in North Taiwan, by which the real dynamic and the characteristics of the network are drawn. The findings of this study are as follows: Before the institutional reform, under the lack of mutual trust and interaction accompanied with a competition mechanism, the OPOs unexpectedly competed for hospitals to join their own networks. After the institutional reform, the incentive structure was changed. The OPOs and some powerful regional hospitals or medical centers formed a new ally. The previous vertical “strong ties” between the OPOs and small hospitals was weakened. It is observed that the networks has transformed from the sort of “competitive” into “collaborative.” During this transformation, the government showed its coercive power to direct the process and enforced a more stringent rules and regulations into the institution. This indicates that the government remains an essential actor in the network, and a public authority and law remains critical. This institutional reform has made the ideals and the practices of organ procurement converge among the OPOs. However, this study predicts that under the new institution, the OPOs, only a small minority of hospitals, will dominate most power and resources in North Taiwan that may in turn cause power struggle within the network and further stratify the hospitals. Lastly, this study suggests that besides the refinement of goals and values of network actors, the government should upgrade its own network governing capability and establish an accountability and evaluation system for the collaborative network, fostering a better relationship among the network actors.
